As a patient in the Seirei Hamamatsu General
Hospital, you have the right to:
- Receive treatment without discrimination
as to your particular set of circumstances including nature
of illness, race, religion, sex, disability, and national origin.
- Receive complete information
about your diagnosis, procedure, and treatment.
- Be informed of all the possible
risks and benefits of the procedure or treatment.
- Refuse treatment and be told
what effect this may have on your health.
- Give informed consent not to
be resuscitated.
- Refuse to take part in research.
In deciding whether or not to participate, you have the right
to a full explanation.
- Complain, without fear of reprisals,
about the care and services you are receiving.

As a patient in the Seirei Hamamatsu General Hospital, it is your responsibility to:
- Provide accurate and complete information about your past illnesses, hospitalizations, medications, and other matters related to your health.
- Inform your physician or nurse if there is a change in your condition or if problems arise during your treatment.
- Provide accurate information related to insurance or other source of payment.
- Understand that it may become necessary to transfer you to another bed within the hospital.
- Honor our No Smoking Policy.
