| Fill the application form at the front desk #100. Make reservation at #100 desk with your application form, health insurance card and, if you have, your consultation card of our hospital. Fill the medical history form at the reception desk of the department you booked. Put the interview sheet into the brown box and wait for a nurse's interview. A registration nurse ask you some questions and after that she/he pass you a same-day appointment ticket. Please visit the department again on the appointment time. |
