Introduction for Consultation

Patients visiting our hospital for the first time and patients whose last visit was more than three months ago must fill out an intake form. However, patients who have an appointment via a medical institution and have a “Referral Patient Appointment Card” do not need to fill one out.

Payments and Scheduling
Here you can make payments for services and schedule your next appointment.

After payments are made, receipts, next appointment cards, prescription exchange tickets (redeemable within hospital), and medical service details can be issued here. Payment via credit card is accepted.
External PrescriptionsWe will issue a prescription slip to you at your examination room, please redeem it at an external pharmacy.

Returning Home

  • Depending on the department, referral letters and/or prior appointments may be required (referral system). Please check with the relevant department before visiting the hospital.
  • We must confirm your health insurance once per month. Please present it at the reception or payments and scheduling desk.

Appointments Made by Patients (First Visit)

It may be difficult to schedule an appointment at your preferred time and/or with your preferred department, as departmental appointment and a check of your physical condition is required. We recommend making an appointment via another medical institution.

1. After obtaining a referral letter (a document providing your medical condition), please call the Regional Medical Care Contact Office (JUNC).

2. The Regional Medical Care Contact Office (JUNC) will inform you of available appointment dates and times.

3. On the day of your appointment, make sure to bring your referral letter and any reference materials with you.

Making Changes to Follow-up Appointments

Changes in follow-up appointments can hinder our ability to offer appropriate medical care, so please refrain from doing so. In unavoidable circumstances, please contact via the following phone number to make appointment changes.
