Initial Medical Examination



About Initial Medical Examination

You will be treated as an initial medical examination if any of the following apply.

  1. When you visit our hospital for the first time or when you see a doctor in a clinical department you are going to for the first time
  2. When a patient has been cured or treated for a disease after a previous visit to our hospital, but is now visiting our hospital because of a new disease that has occurred since the previous visit.
  3. When a person who needs continuous treatment discontinues the treatment at his/her own discretion and returns to see the doctor after a lapse of more than 3 months.
About Referral Letter (Extra Fee for a First-time Patient without a Referral letter)

Patients who do not have a referral letter from another medical institution will be charged a separate fee of 7,700 yen (tax included) for the fee for treatment of patients’ choice, in addition to the first visit fee.

  • On Saturdays, only certain clinical departments are open.
  • Consultation days and consultation start times may differ depending on the clinical department.

No-consultation day

Saturday afternoons・Sundays・Holidays・New Year Season (12/29 to 1/3)

* Beginning in October 2019, the hospital will be closed in all clinical departments with the exception of a few departments.

Flow of Medical Examination

Those with a Referral Letter

1. Appointment

If you have completed an appointment at your regular doctor, please proceed to the 2.
Those with a referral letter, and no appointment

Patients with a referral letter can make their own appointments.
Please have a letter of referral letter (Patient Referral Document) ready at hand and, contact the Office of Regional Care Communications (JUNC).

2. Reception

On the day and time of your appointment, please come to Window 1 [reception desk] located in front of the main entrance with the following items.
【Things to bring that day】
  • Health Insurance card
  • Referral letter
  • Patient registration form

<If possessed>
  • Our medical registration card
  • Various kinds of beneficiary certificate
  • Reference materials such as CD-ROMs
  • Appointment card of Referred patient
  • A resident card(Foreign nationals)

3. Reception at the Clinical Department

Please approach our staff at the reception desk.

4. Medical Examination

5. Fee Calculation

Fees for medical examinations and tests, etc. performed on the day are calculated.
We will also schedule the date and time of your next appointment, and guide you the procedure of hospital visiting.

6. How to pay for expense of medical treatment

Please use the automatic payment machine.
To initiate payment at the automatic payment machine, please scan the barcode on sheet for your outpatient schedule (the paper coming out of machine for reception of re-medical examination) or insert your medical registration card.
Cash or credit cards are accepted.

7. (Those who are prescribed medications)How to receive Medications

For extramural dispensing, please bring your medical prescription to a dispensing pharmacy outside the hospital.
For in-house prescription, please come to counter No. 5 [Medications counter] on the 1st floor of Building B.

8. Completion of Medical Consultation


Those without a Referral Letter

1. Reception

Please come to Window 1 [reception desk] located in front of the main entrance with the following items.
  • Some medical departments require appointments and, some medical departments are not available on certain days of the week.
  • In principle, please completed an appointment at your regular doctor.

Please print these out in advance, fill in the necessary information, and bring them with you to your first appointment.

Please print out the form, write down each item, and bring it with you to your first visit to facilitate your medical examination.
